Responsibilities Provides dementia care including adapted ADLs, meaningful engagement and communication to residents following a person-centered individual service plan. Treats residents with respect and dignity, recognizes individual needs, and encourages independence. Fosters a purposeful and engaging culture throughout the community.
Assists residents and provides input in regards to the care plan and daily living activities, including bathing, dressing, grooming, toileting, transferring and meals. Encourages residents to be as independent as possible.
Participates in team behavioral expression meetings or discussions with community leadership.
Knows the residents’ Life Story, preferences, routines and interests.
Serves meals in the dining room; may assist with meal preparation following dementia-friendly menus. Records and reports changes in eating habits to the supervisor.
Initiates and/or leads at least one Daily Path engagement program; encourages socialization and participation in person-centered activities.
Maintains residents’ records daily in accordance with policy; observes and reports health and emotional changes to the supervisor.
Follows medication policies and, where permitted, assists with medication administration as defined by state regulations.
Maintains a clean, safe, and orderly environment; performs housekeeping and related duties.
Responds to resident requests promptly and follows emergency procedures as needed.
Monitors resident location and notifies supervisor if a resident is missing.
Encourages teamwork and cooperative interactions with co-workers.
Qualifications Education and Experience High school diploma or GED and three to six months related experience preferred, or equivalent combination of education and experience. Knowledge of dementia, particularly Alzheimer's type. Refer to state regulations for requirements related to medications and direct care duties.
Certifications, Licenses, And Other Special Requirements May need to possess current state certification and follow regulations to maintain certification, per state law.
Physical Demands and Working Conditions
Standing, walking, sitting
Use of hands and fingers; reach with hands and arms
Climb, balance, stoop, kneel, crouch, crawl
Talk or hear; taste or smell
Ability to lift up to 50 pounds; vision
Occasional weekend, evening or night work; on-call as needed
Possible exposure to communicable diseases and infections; exposure to latex; potential exposure to blood-borne pathogens
Travel occasionally
