The Family-based Intensive Treatment (FIT) Program provides intensive short-term (on average, 4 to 6 months) in-home, community-based treatment to an identified youth and their family. The focus of FIT is to ameliorate the youth’s mental health issues and strengthen the family structures and supports, with the goal of safety transitioning the youth into a less-intensive, community-based treatment. To do so, at-risk youth and their families are provided medically necessary intensive family therapy and robust care coordination. The clinical goal of this program is to provide youth under 21 and their parents/caregivers with the intensive short-term treatment needed to maintain the youth at home safely and to (re)connect them to ongoing outpatient and/or community-based services. Responsibilities
The Family Partner provides education and support throughout the duration of treatment, attends care coordination meetings, and may assist caregivers in articulating the youth\'s strengths, needs, and goals. The Family Partner educates caregivers about how to effectively navigate the child-serving systems and available informal/community resources, and facilitates access to these resources. This may include: Supporting the parent/caregiver/guardian in navigating the child-serving systems and processes; fostering empowerment, including linkages to peer/parent support and self-help groups. The Family Partner provides care coordination as clinically indicated through phone calls, in-person meetings, and/or virtual meetings with collaterals, including making appropriate referrals and warm hand-offs to follow-up providers, as indicated. The Family Partner FIT Service provider supports identification of community resources and development of natural supports for youth and family to support and sustain achievement of the youth\'s treatment plan goals and objectives. The Family Partner will work closely within a multidisciplinary team that includes a clinical team lead and counselor to provide support and interventions that teach the caregiver to navigate the child-serving system and empower them to do so on their own. Administrative Responsibilities/Record Keeping
Maintain current on all Agency, Government and Funding trainings and health requirements (OSHA, Sexual Harassment, etc.). Complete all necessary paperwork in a strength-based manner per Agency requirements (e.g., court letters, change of placement forms, referrals, ICP, progress notes, evaluation instruments, consent forms). Complete time sheets, mileage forms, and submit them accurately and in a timely manner. Minimum Qualifications
Bachelor’s degree or associate degree in a human services field (preferred) and one year of experience working with the target population, or high school diploma or HISET with a minimum of two (2) years’ experience working with children/adolescents/transitioning youth. Experience as a caregiver of youth with special needs, preferably with youth with mental health issues. Experience with child and family-centered, strength-based services. Experience navigating child and family-serving systems and teaching family members involved with these systems. Demonstrated ability to be innovative, creative, analytical and decisive in problem solving. Strong communications, administrative and organizational capabilities. Ability to work effectively with diverse populations and community agencies (e.g., School Dept., DYS, Juvenile Court). Willingness to be part of a program that supports youth and families 24/7. Must have a driver’s license and access to a vehicle to support program needs. Bi-lingual Spanish/English preferred.
