GPCS is hiring a Full Time Developmental Services Support Coordinator position.
This position is responsible for providing support coordination to support children and adults with Intellectual Disabilities (ID) and/or Developmental Disabilities (DD). Support Coordination includes conducting assessments, developing person-centered plans, and assisting eligible consumers in accessing the most appropriate services to meet their individual needs. This is accomplished through linking, monitoring, documenting, coordinating resources, and advocating for effective and responsive service provisions.
Bachelor’s degree in human servicesand experience with ID/DD population is required.

Salary range is 55,000- 69,031 based on education and experience.
