Certified Occupational Therapy Assistant (COTA) - PRN
Starting Rate: $45/hour
Sequoia Transitional Care, a skilled nursing facility, is seeking a dedicated
PRN Certified Occupational Therapy Assistant (COTA)
to join our rehabilitation team. This role provides flexibility while making a meaningful difference in the lives of our residents. General Purpose
The COTA assists the Occupational Therapist with patient-related activities and direct patient care, helping residents achieve their highest level of independence and quality of life. Essential Duties Provide therapy treatments as directed by the Occupational Therapist. Record daily treatment notes and weekly progress notes in compliance with OT Board standards. Follow physician treatment plans for each patient. Assist nursing staff with training Restorative Aides. Communicate effectively with the supervising OT and interdisciplinary team regarding patient progress and plans. Participate in in-service training programs for facility staff. Maintain accurate documentation including evaluations, recertifications, and discharge notes per state and payer requirements. Provide caregiver training and family education to support safe discharge planning. Assist in maintaining a clean and safe treatment area, reporting equipment issues promptly. Qualifications Current
COTA license
in the state of California. Proficient in electronic documentation systems (e.g., Casamba, PointClickCare). Strong communication skills with patients, families, and healthcare team members. Ability to read, interpret, and apply therapy standards, policies, and physician orders. Must meet ongoing state continuing education requirements. Physical Demands Frequent standing, walking, and reaching. Ability to push/pull and lift up to 50 lbs frequently and 100 lbs occasionally. Occasional stooping, kneeling, or crouching. Visual acuity including close, distance, and peripheral vision. Work Environment Skilled nursing and rehabilitation setting. Low to moderate noise levels.
