Req#: BILIN001190 Position Title : Bilingual Resource Navigator, Enhanced Care Management (ECM) Organization : Jewish Family Service of San Diego Department : Family and Community Services Position Type : Full-Time (37.5+ hours/week), Non-Exempt Work Setting : Hybrid Reports To : Program Manager Pay Rate : $21.84- $25/hour Total Compensation : In addition to standard pay, compensation for this position includes: Comprehensive, low-cost healthcare coverage for employees Generous employer 401(k) contributions Employer-covered life insurance Time Away from Work : Time off benefits for this position include: Generous paid vacation time and sick leave 17 paid holidays, including Federal, Jewish, and floating holidays 2 Wellness Days to support employees mental wellness Position Overview : The Enhanced Care Management Program and Community Supports program provides intensive care coordination services for Medi-Cal members who are experiencing homelessness, are high utilizers of medical emergency services, or are diagnosed with Serious Mental Illness (SMI) and/or Substance Use Disorder (SUD). The Resource Navigator will coordinate care for individuals, assist clients in navigating health care and social service resource systems, and establish manageable goals to improve health care outcomes and quality of life. Responsibilities : Provide comprehensive assessments for individuals, identifying areas of need and helping clients set and achieve goals Provide ongoing case management and supportive services to Enhanced Care Management program participants and their care partners Attend to telephone calls, assessing each situation and screening for high-risk situations Accompany clients to medical and other service-related appointments when necessary Carry a caseload of individuals (estimated 50 households) Work closely with other case managers/case aides and health care partners to coordinate care for shared clients Provide appropriate external referrals as needed Ensure clients access any and all public benefit programs for which they are eligible Work with the client to formulate and execute realistic and time-specific personal goals Maintain accurate and detailed documentation of client interactions in software systems Skills/Abilities That Are a Must-Have : Bilingual (Spanish) High School Diploma required; Bachelors degree preferred or equivalent work experience Minimum of one year of experience working with populations experiencing homelessness, addiction, and/or mental health issues Comfort conducting outreach to clients in the community Ability to work flexible hours, including some evenings Current California Drivers License, proof of insurance, and reliable vehicle Must be able to pass a LiveScan, background check, and drug test Skills/Abilities Wed Like You to Have : Previous training in Motivational Interviewing and Trauma Informed Care preferred Knowledge of community services and resources in San Diego County preferred Important Notice : Incomplete submissions will not be considered.
