Job Description This position ensures organizational excellence related to customer service and quality of care while promoting cultural competence at the assigned hospital. Responsibilities include providing biopsychosocial assessments, psychiatric diagnoses, treatment planning, therapy sessions, discharge planning, and collaborating with interdisciplinary teams to support patient care.
Key Responsibilities
Advanced Clinical Assessment:
Conduct interviews, reviews, and collaborate to interpret biopsychosocial functioning, develop care and discharge plans, and identify abuse or neglect cases.
Advanced Intervention Management:
Develop intervention plans using individual, family, or group modalities, including crisis intervention and supportive counseling, with interdisciplinary collaboration.
Population Management:
Coordinate clinical and financial outcomes, monitor quality of services, liaise with payers, and assist patients in accessing resources.
Documentation and Compliance:
Document assessments and interventions thoroughly, ensuring compliance with standards and regulations.
Additional Duties:
Stay informed about community resources, facilitate linkages, and perform other relevant functions.
Minimum Qualifications Master's Degree in Social Work, licensure as an LISW, and knowledge of medical terminology and disease processes.
