(As seen in the Norfolk Daily News) Social Services/Case Management FULL-TIME 40 hours/week Monday-Friday Essential Duties & Responsibilities Assist the client to find services and providers to meet their needs Conduct in-home visits and follow-up telephone calls for our clients Complete assessments and create care plans for in-home services Qualifications: Bachelor’s Degree in Human/Social Services, Education, or a Registered Nurse with valid Nebraska RN license At least two years’ experience in one of the following fields, long term care, gerontology, rehabilitation, or health/disability case management Position Requirements: Valid Driver’s license Successfully pass a background check Computer experience Northeast Nebraska Area Agency on Aging offers competitive wages, excellent benefit package, paid holidays, and paid leave.
