Overview
Substance Use Disorder Counselor role at Athena Health Care Systems. This is a full-time position located at Summit Commons Rehabilitation and Health Care Center in Providence. Schedule is 8a-4p with weekly pay.
Responsibilities
Provide direct services to residents as represented in the caseload through evaluations and screenings, crisis/emergency assessment and intervention, treatment planning and assessment, and individual and group therapy.
Evaluate residents’ biopsychosocial needs and identify behaviors and barriers that interfere with medical rehabilitation.
Facilitate individual and group counseling and develop appropriate treatment plans to address substance use disorder (SUD).
Participate in interdisciplinary care plan meetings and collaborate with doctors, social services, insurance providers, and hospital networks regarding clients and case management.
Maintain client and administrative records according to Athena policy; attend administrative staff meetings as required.
Provide orientation and ongoing in-service education to staff on the care and treatment of residents with SUD and related subject matter.
Perform risk assessments using current tools to determine appropriate level of care and implement measurable treatment plans relevant to the identified level of care.
Demonstrate clinical expertise specific to the target population (substance abusers in SNF) with the ability to engage clients and collaborate with other professionals and organizations.
Utilize modalities of treatment for SUD in the community to facilitate appropriate discharges (e.g., detox, residential treatment, IOP/SOAP programs, MAT, individual therapy, and self-help programs).
Perform related duties as assigned.
Qualifications
Bachelor’s or master’s degree in psychology, counseling, social work, or related field.
Preferably licensed to practice in the state (LMHC/LICSW/LSW/LMFT/ LADC-I).
Experience in the treatment of clients with a substance use disorder.
Chemical Dependency Provider (CDP) preferred.
