Overview Leading and providing supportive social services to vulnerable populations with mental health/intellectual/developmental disabilities, substance use, and related needs. Responsibilities

Client Support Responsibilities: Bayberry Inc. provides care and supervision to clients with developmental disabilities who reside in licensed community care facilities. Direct Service Professionals (DSP I, II, III) provide care and supervision to clients residing in the licensed community care facilities. Duties include assisting the House DSP I/II/III and opportunities for advancement to a senior DSP position for employees with satisfactory performance. Household chores Food shopping/meal preparation Personal care and hygiene Emergency preparedness Mobility assistance Provide transportation to appointments, including dental, counseling, planning team meetings, etc. Participate in social and recreational outings with clients Maintain awareness of client safety and follow risk management protocol Promote and maintain a positive and supportive environment for the clients Support Team Responsibilities

Participate in planning team process with client, sharing essential information to assist with planning Inform House Supervisor of information to pass on to the facility administrator and outside agencies; House Supervisor communicates to the administrator, regional center CPC and other collaborative agencies as needed Implement Individual Service Plan (ISP) and Individual Program Plan (OPP) Track data per client behavioral plans (IPP/ISP) Participate in additional meetings regarding the client as needed Reporting Responsibilities

Report special incidents to the House Supervisor as soon as they occur Notify supervisor of any changes in client health or activity Notify supervisor of all medication changes per physician order Document daily activity in client file Provide additional documentation as directed by supervisor Bayberry Employee Responsibilities

Communicate professionally with supervisors, coworkers, clients, and their families Establish and maintain professional working relationships with clients, their families, and Bayberry staff Attend required Bayberry meetings Represent Bayberry in a professional manner at all times Perform other duties as assigned Be able to handle crisis situations effectively, and according to protocol Be able to work independently Be able to follow up on directives from supervisor; be dependable and arrive to work on time Be able to read, write, and comprehend English Applicants Must

Be 18 years or older Have a high school diploma or its equivalent Have at least one year of experience working with persons with developmental disabilities Have independent and reliable transportation, along with a valid driver's license, insurance, and clear DMV record Must obtain or provide all necessary clearances to begin work, including: Appropriate fingerprint clearance TB test with negative results (or doctor’s exam for TB positive result) Seniority level
