Case Manager
for our
Partial Care
department. Partial Care provides treatment for the severely and persistently mentally ill population, who may also have substance abuse problems. The program offers assessment and treatment planning, prevocational skill training, medication management, and case management. The
Case Manager
role includes providing case management and support to PC clients, facilitating groups, documenting according to regulatory standards, carrying a caseload, functioning as part of the PC team, interfacing with PC psychiatrists, and maintaining collateral contacts with family members and community providers on behalf of clients.
