Education:
Bachelor of Science in Nursing (BSN) preferred. Licensure:
Current and unencumbered Registered Nurse (RN) license in the state of practice. Experience:
A minimum of two years' experience in a management or supervisory role within a home care or home health setting is highly preferred. Technical Skills:
Strong knowledge of accepted professional standards, Medicare Conditions of Participation, and federal, state, and local regulatory requirements. Soft Skills:
Exceptional leadership, innovation, mentorship, communication, and interpersonal skills. Ability to serve as a key liaison between patients, families, and staff. Key Responsibilities:
Lead and manage the delivery of patient care services, ensuring compliance with all regulatory standards. Provide supervision, mentorship, and support to nursing staff to promote professional development and optimal patient outcomes. Collaborate with interdisciplinary teams to create and implement personalized patient care plans. Ensure precise and timely documentation of patient care and progress. Conduct regular audits of patient records and care plans for quality assurance. Serve as a communication link between patients, families, and healthcare providers to ensure seamless care coordination.
