Position: Admissions Nurse
Location: Bronx, NY
Job Type: Fulltime perm

Responsibility of the Admissions Nurse to participate in the process that assures the clinical, psychosocial, and physical needs of the elderly are met through making appropriate assessments and recommendations. Additionally, the Admissions Nurse makes referrals for placement within the Hebrew Home continuum of care. 
Key Responsibilities: 
Complete clinical assessments and make recommendations to support appropriate placement within our facility.
Review and analyze medical records and electronic referral systems (Allscripts, CarePort, faxed referrals) for accuracy and completeness.
Communicate with referral sources, hospital discharge planners, patients, and families.
Coordinate with social workers and other staff to support timely and appropriate admissions.
Verify insurance, obtain authorizations, and initiate insurance cases as needed.
Serve as a knowledgeable resource on Sub-Acute and Long-Term Care programs within the Hebrew HomeQualifications:
New York State RN license and 3 years of clinical nursing experience, preferably in geriatric care or LPN with at least 5 years of documented geriatric care experience.
Experience in a skilled nursing facility is strongly preferred. Strong assessment skills; critical care experience a plus.
Excellent communication (written and verbal), time management, and organizational skills.
Ability to interpret clinical and psychosocial needs and align them with the appropriate level of care.
