PAY RATE: $35 - $41/HR
Job Summary:
The Charge LVN is responsible for overseeing daily nursing care in our memory care community, providing leadership and support to caregiving staff while ensuring that residents living with Alzheimer’s disease and other dementias receive safe, high-quality, and compassionate care. This position combines hands-on patient care with shift leadership duties in compliance with California Title 22 and Department of Social Services regulations.
Key Responsibilities
Clinical Care
Deliver direct nursing care, including administering medications and treatments according to physician orders and residents’ care plans.
Observe and monitor residents for changes in condition and report findings to the RN Manager or physician promptly.
Perform nursing procedures such as wound care, vital signs monitoring, and infection control measures.
Assist with resident admissions, discharges, and transfers as needed.
Shift Leadership
Supervise, guide, and support CNAs and caregiving staff during assigned shifts.
Delegate tasks appropriately and ensure that daily care routines are completed according to residents’ care plans.
Coach and mentor staff to maintain high standards of dementia care.
Communicate relevant shift information to the RN Manager and incoming shifts.
Resident & Family Communication
Build positive relationships with residents and their families, addressing concerns and providing updates when appropriate.
Promote and protect resident dignity, rights, and quality of life.
Compliance & Documentation
Accurately document care provided and resident status in charts and electronic medical records.
Ensure nursing care meets California Title 22 requirements and facility policies.
Participate in staff meetings, in-service training, and quality assurance programs.
Qualifications
Current, unrestricted Licensed Vocational Nurse (LVN) license in the State of California.
Current CPR/BLS certification.
Minimum of 1–2 years of nursing experience in a skilled nursing, assisted living, or memory care setting preferred.
Previous experience supervising direct care staff preferred.
Knowledge of dementia care practices and behavior management is strongly desired.
Strong communication, leadership, and teamwork skills.
Working Conditions
Work is performed in a licensed residential memory care facility.
May require evening, weekend, or holiday shifts.
Physical requirements include standing, walking, bending, lifting, and assisting residents as needed.
