Position Summary
Lions & Bears Staffing Partners is seeking a Nurse Practitioner with a focus to support a Skilled Nursing Facility in Rockford, IL and surrounding areas. This part-time or full-time opportunity is perfect for providers who enjoy working independently and want to make a meaningful impact in long-term care through proactive infectious disease management.

Responsibilities

• Conduct assessments, diagnose and treat infectious diseases in SNF patients

• Collaborate with interdisciplinary teams to create and update care plans

• Round weekly and manage visit schedules independently

• Maintain detailed and timely clinical documentation

Qualifications

• Active, unrestricted Illinois Nurse Practitioner license

• Master’s degree in Nursing (MSN) from an accredited program

• 2+ years of experience in a Skilled Nursing Facility or long-term care setting (preferred)

• Excellent clinical judgment, communication, and documentation skills

• Comfortable working autonomously in a fast-paced care environment
