Advanced Practice Provider (Nurse Practitioner)

Northern Counties Health Care is seeking a dedicated and patient-focused Nurse Practitioner to join our team at Island Pond Health Center. This full-time position offers an opportunity to serve a vibrant rural community while working alongside a collaborative and mission-driven care team.

Responsibilities:

• Provide comprehensive primary care services to patients of all ages, emphasizing health promotion, continuity of care, and disease prevention.

• Perform assessments, physical exams, diagnostic testing, and manage acute and chronic conditions in accordance with clinic protocols.

• Document care plans and maintain problem-oriented medical records using Electronic Medical Records (EMR).

• Coordinate patient referrals and ensure continuity of care through ongoing collaboration with specialists.

• Participate in community health education initiatives and staff development activities.

• Support continuous quality improvement efforts, including performance audits and protocol evaluation.

• Mentor clinical and non-clinical staff and contribute to a culture of professional development.

• Engage in continuing education to maintain licensure and certification requirements.

Qualifications:

• Education: Graduate of an accredited Nurse Practitioner program; certified and licensed in Vermont with an active DEA license.

• Experience: Prior experience in primary care preferred; a minimum of one year of experience providing care to all age groups if not a Family Nurse Practitioner.

• Skills: Strong interpersonal communication, clinical judgment, and EMR proficiency. Must be team-oriented and committed to community-based care.

• Physical Demands: Must be able to lift up to 50 lbs, stand for extended periods, and respond effectively in emergency situations.
