Location: Wabash, IN • Hours: Mon-Fri 8a-4p Flex (40 hrs.) • Starting Pay: $24.57-28.48/hr • Job Type: Full-Time

Responsibilities

  • Health & Wellness Coordination: Manage and coordinate medical care for individuals supported in group homes (SGL) and residential programs (RHS).
  • Health & Wellness Coordination: Partner with physicians, specialists, pharmacists, and care teams to meet health needs.
  • Health & Wellness Coordination: Develop, update, and maintain individualized health risk and wellness plans.
  • Medical Care & Documentation: Prepare medical reports, progress notes, and risk plans.
  • Medical Care & Documentation: Conduct nurse physicals, quarterly reviews, and ensure proper medical charting.
  • Medical Care & Documentation: Monitor and maintain accurate medication records and related documentation.
  • Medication Oversight & Compliance: Oversee safe medication administration and provide staff guidance.
  • Medication Oversight & Compliance: Perform monthly audits of medication storage and documentation.
  • Medication Oversight & Compliance: Ensure compliance with the Indiana Nurse Practice Act, Medicaid Waiver, CARF, and ISDH standards.
  • Training & Support: Provide training to staff in CPR, First Aid, AED, Core A & B medication administration, and health-related topics.
  • Training & Support: Educate staff, individuals supported, and families on health care practices and high-risk plans.
  • Training & Support: Mentor and consult with team members on medical matters to strengthen overall care.
  • Team Collaboration & Advocacy: Serve as a member of each individual’s interdisciplinary team (IDT).
  • Team Collaboration & Advocacy: Participate in care planning meetings, transitions, and discharges.
  • Team Collaboration & Advocacy: Advocate for people supported, ensuring their voices are heard in health decisions.

Qualifications

  • Current license in the State of Indiana as issued by the Indiana State Board for Registered Nurses or Licensed Practical Nurse is required.
  • Demonstrated ability to support people with disabilities.
  • Knowledge and effective use of electronic documentation, reporting, communication, etc.
  • Ability to project a professional image while engaging with community partners, healthcare providers, and individual support teams.
  • Crisis prevention, intervention, and teaching skills.
  • Valid driver’s license, insured vehicle, and safe driving record.
  • Preferred minimum of three (3) years of experience in community health/mental health services.
  • Ability to speak, read, and hear the English language.
