Home Health Field Registered Nurse (RN) Supervisor

Job Summary

Sunshine Homecare Services is seeking a dedicated and experienced Field Registered Nurse (RN) Supervisor to oversee the provision of home care and NHTD and TBI Waiver services to patients at home. This role is responsible for conducting initial and follow-up assessments in patients homes, developing, implementing and monitoring plans of care, managing and supervising HHA/PCA staff, reviewing and updating care plans, collaborating with service coordinators and physicians, maintaining compliance with state, federal, and agency standards, and fostering a supportive environment for patients, families, and team members. The RN Field Supervisor serves as a clinical resource, monitors patient outcomes, and ensures timely documentation.

Position Details
  • Location: Rockland, Westchester, Orange, Dutchess, Putnam, NYC
  • Employment Type: Full-Time / Part-Time/Per-Diem
  • Schedule: M-F 8:30am-4:30pm
Key Responsibilities:
  • Conduct home visits to patients for initial assessment, follow-up and supervisory visits in accordance with the physician plan of care and regulatory and agency requirements.
  • Develop, implement, review and update care plans to reflect changing patient needs.
  • Carry out plans of care as established by the physician.
  • Monitor patient progress and collaborate with physician, waiver service coordinators and other professionals in the provision of care.
  • Supervise HHA/PCA staff.
  • Ensure compliance with HIPAA and confidentiality standards.
  • Maintain accurate and timely documentation in accordance with regulatory and accrediting body requirements and agency policies.
  • Participate in quality assurance activities, reviews, regulatory visits, and certification processes.
  • Participate in after-hours/weekend On-call on a rotation basis
  • Participate in agency and service coordination meetings as required
  • Participate in the orientation and training of new nurses and aides as needed

Qualifications
  • Current and valid Registered Nurse (RN) license in the State of New York.
  • Nursing degree from an accredited program.
  • Current CPR certification.
  • UAS certification is required for this role and can be obtained while training
  • RN experience in LHCSA/CHHA
  • Pediatric care experience a plus.
  • Reliable transportation.
  • Strong communication skills (verbal and written).
  • Experience with HHAeXchange preferred but not required.
