Position Title: Primary Care Provider (Nurse Practitioner)

Department: Clinical Services
Reports To: Medical Director
Employment Type: Full-Time, Exempt
Location: Esperanza Health Center – Multi-site (North Philadelphia)

Position Summary

The Primary Care Provider (NP) at Esperanza Health Center provides holistic, patient-centered care rooted in Christian values. This role is responsible for timely and effective clinical care, collaboration with a multidisciplinary team, spiritual outreach, and participation in organizational initiatives that promote wellness, efficiency, and patient empowerment.


Key Responsibilities???? Clinical Excellence & Team-Based Care
  • Arrive and start sessions on time; prepared for AM/PM patient sessions

  • Participate in and lead daily team huddles

  • Delegate effectively to support staff and maintain patient flow

  • Follow up on hospital discharges, test results, referrals, and medication refills

  • Provide efficient, high-quality patient care, completing documentation within 24 hours

  • Engage in cost-effective, evidence-based care aligned with EHC clinical guidelines

  • Prescribe responsibly, especially regarding controlled medications

  • Handle patient needs efficiently during on-call periods and complete follow-up documentation

???? Patient Engagement & Empowerment
  • Educate and empower patients to take an active role in their own wellness

  • Schedule follow-ups strategically for chronic condition management

  • Set clear visit agendas and boundaries with patients

  • Provide culturally sensitive and spiritually integrated care

  • Encourage patient involvement in decision-making and follow-up compliance

???? Documentation & Follow-Up
  • Complete charting, referrals, and documentation on the day of the visit whenever possible

  • Review labs and communications bins regularly

  • Address refill and message requests within 2 working days

  • Complete patient forms within 5 working days


Spiritual & Holistic Care
  • Provide spiritual care through prayer, scripture, and connection to churches when appropriate

  • Refer patients to behavioral health or social services as needed

  • Serve all patients with humility, love, and respect—regardless of spiritual interest or beliefs

  • Participate in daily staff devotions and contribute to the spiritual life of the team


Collaboration & Culture
  • Resolve conflicts directly and respectfully with team members

  • Encourage team morale and model a positive, constructive attitude

  • Train, mentor, and precept students when appropriate

  • Suggest and support clinic-wide quality improvement efforts

  • Participate in fundraising and community outreach events (optional)


Clinical Skills & Accountability
  • Demonstrate sound clinical judgment and stay current with care guidelines

  • Maintain up-to-date credentials (license, DEA, CME)

  • Seek supervision appropriately and adhere to licensing limitations

  • Follow organizational protocols, including on spiritual care boundaries and biblical ethics


Qualifications
  • Licensed Nurse Practitioner (NP); Family Medicine preferred

  • At least 2 years experience in primary care

  • Demonstrated commitment to Christian values and ethical practice

  • Passion for serving underserved populations

  • Strong communication, teamwork, and organizational skills

  • Ability to integrate faith into clinical care in a respectful, patient-centered manner

  • Accountable to and active in a Bible-teaching church congregation


COVID-19 Vaccine Requirement

Esperanza Health Center requires all on-site workforce members, including employees, contractors, and volunteers, to be vaccinated against COVID-19. Exemptions for valid medical or religious reasons may be requested. Start dates may be delayed until vaccine compliance is confirmed.
