Position: Residential Nurse (RN)
The RN assesses health-related needs, monitors ongoing health and wellbeing of adolescents being assessed for, participating in or completing treatment at Outreach House, and provides on-call nursing when on site coverage is unavailable, serving as the health care coordinator.
Responsibilities
- Conduct and/or monitor all medication calls and health calls as scheduled.
- Work with clients and guardians to schedule medical appointments and follow up as required.
- Coordinate with the Agency psychiatrist re: client medication and pharmacy refills to ensure prescriptions are filled in a timely manner; contact guardians regarding medications and changes; assist with Order Connect data entry and monitoring.
- Provide on-site health services to address health needs of adolescent clients.
- Provide training and guidance to staff to deal with emergency health situations.
- Establish and monitor health-related policies and procedures with facility managers to ensure a safe environment.
- In a health emergency, provide immediate first aid and monitor until emergency services arrive, escorting clients to emergency or health facilities when necessary.
- Conduct health assessments (nursing intake/health history, tobacco use history and cessation plan, and communicable disease risk) to assess treatment participation suitability.
- Be present during physical exams when requested by the medical director.
- Follow up on internal and external health-related referrals (e.g., intake physicals, health referrals, psychiatric and medical emergencies).
- Maintain a tracking system for client contacts, health issues, medical appointments, prescribed medications (internal/external) and OTC meds.
- Communicate health-related issues to appropriate staff and document incident reports as required by policy.
- Create, organize and facilitate health and wellness activities, including 1:1 health education and counseling, groups and seminars for residents on topics such as diet, exercise, stress management, smoking cessation, sexually transmitted diseases and pregnancy prevention.
- Provide staff education related to health responsibilities and medications processes per Federally regulated standards.
- Prepare health alert documentation as required.
- Facilitate training for staff on crisis intervention related to medical issues when indicated.
- Participate in agency meetings and quality improvement activities.
- Ensure all medical forms meet state and federal compliance requirements and complete required documentation in a timely manner.
- Maintain communication with management about client, staff or programmatic issues and follow emergency contact procedures when appropriate.
- Collaborate with clinical staff to integrate and document RN progress notes and related chart entries.
- Uphold federal and state program administration guidelines for nursing practice; retain and renew RN license every 3 years and provide a copy to HR; report safety concerns promptly.
- Participate in unit fire and safety drills and maintain CPR/FA/AED certifications; provide emergency care as needed.
Supervised by: Medical Director in collaboration with the Program Director.
Qualifications
- NYS Registered Nurse (RN)
- RN with a minimum of 2 years of experience working as an RN in a mental health and/or substance use disorder treatment setting
- Previous experience working as an RN in a residential substance use disorder treatment setting preferred
- Previous experience working as an RN in a related treatment, therapeutic community, community residence, and/or mental health setting working directly with special populations required
The schedule for this position is Mon-Fri, 3:00-11:00 PM.
Work environment: This full-time, non-exempt position operates in a professional office environment and outdoors; routine use of standard office equipment. Travel among assigned sites may be required.
