Summary: Enroll, coordinate services/ referrals, and provide comprehensive nursing care via in-person and/or virtual visits to a caseload. Monitoring client vitals, recording changes in conditions, taking patient histories, and organizing health records. Visits may include nursing assessments, education, guidance, and support to families in their homes, locations of their choice, or via telehealth.

Duties and Responsibilities:

  • Assessing patients: Home visiting nurses evaluate patients' well-being.
  • Maintaining records: Home-visiting nurses maintain accurate records of the services they provide.
  • Connecting with care providers: Home-visiting nurses collaborate with other providers and physicians.
  • Monitoring vitals: Home visiting nurses monitor and record patient's vital signs, when necessary.
  • Developing care plans: Home-visiting nurses help develop and maintain patient care plans.
  • Documenting visits and submitting information within the specified time frame.
  • Performs other related duties as assigned.

Skills/Licenses/Certifications Necessary:

  • Registered Nurse, ADN/ASN
  • BLS current certification
  • Excellent verbal and written communication skills.
  • Excellent interpersonal and customer service skills.
  • Excellent customer service skills.
  • Excellent organizational skills and attention to detail.
  • Excellent time management skills with a proven ability to meet deadlines.
  • Strong analytical and problem-solving skills.
  • Ability to prioritize tasks
  • Ability to function well in a high-paced and at times, stressful environment.
  • Proficient with Microsoft Office Suite or related software.

Preferred Qualifications:

  • BSN
  • CLC/IBCLC Certification
  • CPR
  • Bilingual

Education/Experience:

  • Bachelor of Science in Nursing, Registered Nurse who will complete BSN within one year
  • Experience in Maternal Child Health, Pediatric Health, Mental Health, and/or case management is a plus.

Physical Requirements/Working Conditions:

  • Must be able to drive around county to complete visits
  • Must have private area to contact clients and discuss protected health information
  • Must be able to lift up to 15 pounds at times.
