Licensed Practical Nurse (LPN) – ACT Team | Bronx, NY | Flexible, Hybrid WFH Schedule | $33/hour + Excellent Benefits

Schedule:
- Full-Time: 35 hours/week | Choose your own schedule | Hybrid Remote
Position Overview:
This is a community-based/mobile position with a flexible, hybrid WFH schedule of 35 hours/week—you’ll travel throughout the Bronx providing vital support to a high-need population living with serious and persistent mental illness. Each day includes in-the-field visits with 1–6 clients, followed by remote documentation from home. The ideal LPN is resourceful, flexible, and deeply committed to underserved communities.
LPN (Licensed Practical Nurse) Key Responsibilities:
- Deliver high-quality mental health nursing services in community settings (homes, shelters, public housing, etc.)
- Administer long-acting injectables (LAIs), vaccinations, IMs, and PPDs
- Provide medication education, vital sign monitoring, and documentation in the EHR
- Collaborate with RN, psychiatry, and multidisciplinary ACT team members
- Assist clients in accessing primary care, housing, income benefits, substance use and mental health services
- Participate in virtual morning meetings, with occasional in-office pickups of supplies or in-person check-ins
- Complete detailed, timely progress notes and medication records
LPN (Licensed Practical Nurse) Qualifications:
- Current NYS Licensed Practical Nurse (LPN) license required
- Minimum 2 years’ experience in psychiatric/mental health nursing (experience with SPMI, substance use, or community mental health strongly preferred)
- Comfortable working in field-based, mobile roles
- Strong documentation and organizational skills
- Bilingual (English/Spanish) candidates highly preferred
