Job Description

The Medical Social Worker will provide advanced psychosocial evaluations, counseling, and discharge planning services in an inpatient hospital setting. This role is part of an interdisciplinary care team, supporting both pediatric and adult patients, depending on assignment preference. Candidates must be comfortable managing a high patient volume and collaborating with nursing case managers in a fast-paced medical environment.

Responsibilities / Job Duties:
  • Perform comprehensive psychosocial assessments for hospitalized patients and families
  • Provide therapeutic counseling related to illness adjustment and care plan adherence
  • Develop and coordinate discharge plans, including referrals to appropriate community resources and care facilities
  • Attend and contribute to interdisciplinary team rounds and case conferences
  • Serve as a mentor to social work students and entry-level social workers
  • Maintain accurate and timely documentation as required by the Social Work Department
  • Participate in professional development and hospital/community liaison activities as needed
Minimum Education and Experience Qualification Requirements:
  • Master's Degree in Social Work (M.S.W.) from a CSWE-accredited program - Required
  • Valid New York LMSW or LCSW license - Required and must be maintained
  • 2+ years of social work experience in a hospital or clinical setting - Preferred
  • Experience with discharge planning and interdisciplinary teams - Preferred
  • Strong communication, crisis intervention, and documentation skills
