Qualifications and description for a Social Worker, LSW / CSW:

• NJ Social worker, CSW / LSW License
• Conduct Social Service assessments and documents according to state and federal regulations
• Provide Psychosocial assessments and relevant parts of Minimum Data Set (MDS)
• Counseling Residents
• Subacute experience highly preferred
