The referenced pay range represents the minimum and maximum compensation for this job. Individual annual salaries/hourly rates will be set within the job's compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations.
Responsibilities
GENERAL PURPOSE
The social worker provides consultation and clinical social work services to URMC Primary Care Network patients and their families pursuant to the PCN Patient Centered Medical Home. Social work services are provided in a centralized model across primary care offices and is not embedded in any one office. The social worker collaborates with the health care team to assist patients identified as needing social work intervention. The social worker provides consultation to health care providers regarding patient care needs/community resources and documents all information in the patient’s electronic medical record. The social worker may meet with the patient in the primary care setting to offer further assessment and engagement. May make home visits as clinically necessary.
Essential Functions
Social Work Assessment
- Assesses needs of patients and families referred by care managers and providers.
- These patients include but are not limited to patients across the lifespan in need of resource assistance or protection; persons experiencing emotional crises; and patients with new, chronic or terminal diagnoses.
- Assesses patient and supports patients with complex SDOH needs that impact access to care in the primary care setting.
- Works collaboratively with patient/family natural resources as well as community supports to address needs.
- Assists in the identification and management of possible child maltreatment, IPV, elder abuse and abuse of other vulnerable patients per Social Work Division policies.
- Provides social work services to address needs including complex SDOH issues across primary care practices.
- Other services include crisis intervention, transitional, long-term care and respite care planning, end of life support, adjustment to diagnosis and supportive counseling, information and referral, resource mobilization and advocacy as indicated.
- Coordinates referrals to and acts as liaison to community agencies regarding services to individual patients and their family and does so in collaboration with additional interprofessional care team members as needed.
- Provides internal and external systems advocacy as indicated for patients of the PCN Primary Care practices.
- Maintains current knowledge of community resources relevant to the PCN primary care patient population.
- Documents all social work activities in the patients' medical record in accordance with the relevant Social Work Division policy and procedure.
- Participates in SWD and PCN meetings and activities including but not limited to timely completion of statistical reports/forms, time reporting and other PCN or Division forms, mandatory in-service training, and UHS.
- Participates in social work supervision/consultation about his/her work and professional development plan.
- Participates in relevant professional development trainings.
- Meets with PCN administrative and care management teams as required.
- Offers social work consultation to other care team members regarding patient care management needs and community services.
- May support orientation and ongoing training to PCN care managers and providers regarding social work services and selected psychosocial problems and/or resources.
- May provide field work instruction to BSW/MSW social work students after one year of employment and with approval from AHP and SW supervisor.
- Provides coverage services as requested during the absence of co-workers.
Minimum Education & Experience
- Master's in social work from an accredited program Required.
- Experience in a medical or clinical setting Required.
- Strong interdisciplinary, collaborative skills Required.
- Social Work licensure required within 18 months Required.
