About the Role:

The Adult Prevention Team focuses on early intervention, promoting independence, and preventing the escalation of needs for adults. The Social Worker will provide a person-centred approach to assess and address individuals’ needs, working collaboratively with them, their families, and other professionals to enable sustainable outcomes and reduce reliance on statutory services.

The role requires in-depth knowledge of relevant legislation, such as the Care Act 2014, Mental Capacity Act 2005, and Safeguarding Adults Framework, and the ability to apply strength-based, preventative approaches in practice.

Main responsibilities:

• Undertake person-centred assessments, focusing on strengths and preventative measures to meet individual needs.
• Work with adults, carers, and families to develop support plans aimed at promoting independence and wellbeing.
• Ensure eligibility criteria under the Care Act 2014 are applied fairly and consistently.
• Provide information, advice, and signposting to community resources and voluntary organisations.
• Identify opportunities to prevent or delay the escalation of care needs through timely interventions.
• Promote and enable independence, connecting individuals with appropriate services and support.
• Work collaboratively with health, housing, and community partners to deliver holistic and innovative solutions.
• Identify and address safeguarding concerns, ensuring individuals are protected from harm and abuse.
• Undertake safeguarding enquiries under Section 42 of the Care Act 2014 and develop safeguarding plans where necessary.
• Promote a safeguarding culture, working with individuals and multi-agency partners to reduce risks.

Requirements of the Social Worker:

• Qualification: Degree or Diploma in Social Work and registration with Social Work England.
• Experience: Previous experience in adult social care or similar settings.
• Knowledge: Strong understanding of UK legislation, including the Care Act 2014, the Mental Capacity Act 2005, and safeguarding procedures.
• Skills: Excellent communication, assessment, and decision-making skills. Ability to work autonomously and as part of a multidisciplinary team.
• Other: Willingness to travel for home visits and meetings, where required. A full UK driving licence and access to a vehicle are desirable.
