Overview
We\'re Hiring! Progressive Community Health Centers is seeking an experienced professional to serve as a Social Worker at our Lisbon Avenue Health Center. Under the direct supervision of the Director, Behavioral Health and supported by the Behavioral Health Consultant (BHC) team, the Social Worker is responsible for managing, educating, and referring high-risk patients with chronic illness or disease.
Founded in 1999, Progressive Community Health Centers is an independent non-profit organization that operates four clinic locations in Milwaukee, Wisconsin. More than 100 employees support our agency\'s mission to improve the health and quality of life of the community by providing culturally competent services that address identified needs. We provide high-quality, comprehensive care to roughly 16,000 patients annually, the majority of whom are low-income. We never refuse care based on insurance status or ability to pay. We value a patient-centered philosophy and provide a range of services including family medicine, internal medicine, women\'s health, urgent care, dental, radiology, and integrated behavioral health, along with supportive services such as case management, health education, and benefits enrollment.
What will you do?
The social worker assists PCHC patients by creating linkages to care and providing education and self-management support. Care linkages can include navigation to social support agencies based on identified needs or resources to mitigate the impact of social determinants of health. Schedule: Monday - Friday, 8 a.m. - 5 p.m.
Responsibilities
- Collaborates with PCHC providers in identifying appropriate patients needing social work services.
- Conducts initial and periodic assessments with patients referred to a Social Worker. Prioritizes patients according to intensity, need, and required follow-up.
- Formulates and implements a plan that addresses the patient\'s identified needs by assessing the patient/family needs, issues, resources, and care goals; determining the choices available to individual patients; and educating the patient/family on available options.
- Establishes a plan mutually agreed upon by the health care team and the patient/family. Evaluates the plan\'s effectiveness in meeting established care goals; revises it as needed to reflect changing needs, issues, and goals.
- Identifies and effectively utilizes community resources to meet the needs of patients/families.
- Promotes patient self-management and empowers patients and families to achieve maximum wellness and independence. Assists patients with items such as transportation, scheduling, housing, and food sources.
- Assists the patient in identifying and/or developing their support system and encourages them to utilize it.
- Maintains accurate and timely documentation.
- Participates in departmental and organizational committees as applicable.
- Promotes collaborative teamwork.
- Maintains appropriate professional boundaries.
- Contributes to developing a social work program and plans for future expansion.
Education And Experience
Required Qualifications
- A master\'s degree in social work from an accredited program is preferred, but applicants with a bachelor\'s in social work are welcome to apply.
Required Licensing
- Licensed in Wisconsin as a Social Worker (APSW or obtain APSW within the first six months of employment if a Master level social worker).
Related Work Experience
- Experience preferred with some clinical exposure, preferably in public health or community health.
