1+ Year Social Work Experience

Approximately 10% of all dialysis patients reside in a SNF and are transported to dialysis three times a week placing a significant burden on patients and adding significant avoidable costs to the health care system. SNF dialysis patients receive dialysis treatment onsite at the Skilled Nursing Facility in which they reside.

The Social Worker will support DaVita Skilled Nursing Facility Dialysis patients. The Social Worker must be able to visit each site.

The SNF dialysis locations supported include:

• Guggenheimer Health & Rehab Center, 1902 Grace St, Lynchburg, VA 24504
• Highland Ridge Rehab Center, 5872 Hanks Ave, Dublin, VA 24084
• Charlotte Health and Rehab Center, 1735 Toddville Rd, Charlotte, NC 28214

Requirements:

• Meets all state required regulations to practice in the dialysis setting.
• Master's degree in Social Work (MSW) required with a specialization in clinical practice
• Demonstrated knowledge of government and private insurance programs
• Basic computer skills in MS Word, Excel, PowerPoint and Outlook as well as functional proficiency with DaVita specific applications within 60 days
