Overview

Hospice Social Worker - Full Time. The Parker Jewish Institute for Health Care and Rehabilitation is seeking a Social Worker to join the Hospice department. This position includes field visits in Queens and Nassau counties, in addition to in-patient visits.

Responsibilities

• Deliver appropriate social work services to assigned patients, residents, family members and significant others under the supervision of the Hospice Administrator.
• Perform initial psychosocial, emotional, spiritual and bereavement assessments; conduct ongoing assessments; assist in the development and implementation of goal-directed IDG care plans; advocate for patients and families.

Position Qualifications

• Master\'s Degree in Social Work
• Active NYS Social Work License
• Prior experience as a Social Worker in a community health, hospice or inpatient setting
• Valid New York State driver\'s license and ability to make field visits in Queens and Nassau counties
