Responsibilities

• The Social Worker is responsible for assessing, counseling, interviewing, and educating clients and their family members, and assisting in planning, organizing, and implementing facility policies and procedures to ensure medically related emotional and social needs are met on an individual basis.
• Assess nature of patients' situations by interviewing and reviewing personal history.
• Plan, coordinate, manage, and implement support packages to help patients/residents deal with difficulties and overcome dependencies.
• Develop and execute individual treatment plans and assist with Medicaid and Medicare applications for the patient.
• Assist patients with Food Stamps, Housing applications, and phone assistance.
• Interview service users to assess current condition, needs, strengths, and weaknesses.
• Set tailored, measurable goals for each case and monitor progress, adjusting treatment plans as needed.
• Offer information and counseling on the best course of action during sessions; facilitate referrals to other healthcare professionals and programs.
• Maintain accurate patient documentation and act as a key-worker in cooperation with multidisciplinary teams.
• Promote patient/resident rights and maintain privacy, dignity, and well-being in a safe environment.

Qualifications

• LSW or LCSW is required.
• Bachelor's degree in social work or related field.
• Must have a reliable source of transportation.
