Essential Functions
- Delivery of Patient Care
- Conduct psychosocial assessments of patients/caregivers and families to identify emotional, social, and environmental strengths and problems related to their diagnosis, illness, treatment, and life situation.
- Develop, implement, and evaluate plans of care for patients/caregivers and families. Address counseling, education, resource/referral services, advocacy, and culturally sensitive care. Update plan of care as goals are achieved or changed.
- Support patient/family’s spiritual/cultural beliefs by assessing cultural issues and collaborating with interdisciplinary teams to provide culturally sensitive care.
- If not an LCSW, consult with a Licensed Clinical Social Worker or Social Work Preceptor for clinical interventions beyond scope of practice.
- Document assessments, care plans, interactions, and interventions according to regulatory and agency standards.
- Participate with the interdisciplinary group (IDG) and other VIA departments to achieve patient care goals.
- Provide education on hospice philosophy and services, advance care planning, and end-of-life care issues.
- Conduct family meetings and collaborate with long-term care staff to determine appropriate level of care and service utilization.
- Initiate referrals for volunteers, chaplains, and grief services; reassess supportive service needs over time.
- Maintain knowledge of community resources and refer patients/families as appropriate.
- Manage time, caseload, and technology effectively.
- Assumes responsibility for the effective and professional delivery of health services.
- Schedule visits based on caseload priorities; prepare for visits, review plan of care, and obtain needed materials and resources.
- Conduct visits, follow the plan of care, evaluate patient/caregiver responses, and revise the plan as needed.
- Provide interventions aligned with the plan of care and identify new problems or needs as they arise.
- Facilitate transfers to other settings when appropriate; encourage patient and caregiver independence.
- Support transition from active care to grief care services after a patient’s death.
- Plan daily work to address priorities and minimize travel time.
- Documentation and Compliance
- Ensure accurate, complete, and timely clinical documentation in accordance with VIA guidelines.
- Keep clinical records current by updating electronic medical records; submit documentation per guidelines; maintain time/mileage and activity logs per policy.
- Interdisciplinary Collaboration and Growth
- Establish and maintain effective working relationships with interdisciplinary teams and interdepartmental staff.
- Attend and participate in meetings; contribute to continuous improvement efforts; maintain open communication with supervision and escalate complex issues as needed.
- Engage in ongoing professional development and stay current with trends in social work; maintain licensure as applicable (NC LCSWA/LCSW; SC LISW-CP) and remain in good standing.
Qualifications
May perform other duties as required.
Minimum Qualifications
- Bachelor’s degree required for a BSW role.
- Master’s degree in social work from a CSWE-accredited program for MSW roles; medical social work or hospice experience preferred.
- Current licensure as a Clinical Social Worker Associate (LCSWA) or Clinical Social Worker (LCSW) in NC, and Licensed Independent Social Worker – Clinical Practice (LISW-CP) in SC, and remain in good standing with the licensing boards.
