Social Worker – Care Neighborhoods

Job Objective

Reporting to the Care Neighborhoods Administrator, the Care Neighborhoods Social Worker supports care coordination by recognizing residents, families, medical teams, and staff as essential partners. The Social Worker identifies psychosocial, mental, and emotional needs, facilitates access to services, and serves as Admissions Coordinator for the Care Neighborhoods.

Essential Duties

Admission Coordination

  • Manage the entire admission process, including evaluating needs, reviewing insurance, conducting tours, interviewing, and completing paperwork, ensuring regulatory compliance and positive referral relationships.
  • Support occupancy stability by anticipating openings, working with marketing for external referrals, and maintaining bed and referral trackers.
  • Ensure understanding and signing of admission packets, develop social histories, and orient residents and families to Saint John’s services and rights.
  • Assist residents and families with coping, provide daily support for new admissions, and monitor hospitalized residents for potential admission.

Communication, Planning, and Documentation

  • Complete assessments and maintain documentation within the electronic medical record, adhering to standards.
  • Coordinate care conferences and work with residents and families on advance directives.
  • Ensure progress notes meet regulatory standards and serve as a resource on Medicare and Medicaid benefits.

Psycho-social and Behavioral Health

  • Facilitate access to psychosocial resources, conduct screenings, and help with coping and grief processes.
  • Coordinate behavioral health services and manage concerns and grievances.

Discharge Planning

  • Manage resident transitions, coordinate post-discharge services, and connect residents with community resources.

Professionalism

  • Maintain confidentiality, uphold resident rights, lead investigations, and collaborate with staff, residents, and families.
  • Complete mandatory training, attend meetings, and foster person-centered care and professional integrity.

Knowledge, Skills, and Qualifications

  • Knowledge of relevant regulations, community services, and the MDS system.
  • Proficiency in Microsoft Office and excellent communication skills.
  • Dependability, ability to work flexible hours, and ability to manage workload efficiently.
  • Licensed as a Social Worker in Wisconsin, with a Master’s degree preferred, and at least 2 years of geriatric or rehabilitation experience, including with dementia and behavioral health residents.

Work Environment

Indoor work in a well-lit, climate-controlled environment with typical office equipment.

Physical and Sensory Requirements

Ability to lift up to 10 pounds, perform manipulative tasks, navigate the area, and communicate effectively. Must handle stress and maintain focus amid interruptions.

Emergency Responsibilities

Understand and comply with emergency procedures, assist with resident evacuation, and adhere to safety policies.
