Salary: $20.50 - $25.56 / hourly
Responsibilities
- The social worker will work with residents in the nursing home by identifying their psychosocial, mental, and emotional needs, and providing, developing, or aiding in the access of services to meet those needs.
- Foster a climate, policies and routines that enable residents to maximize their individuality, independence, and dignity.
- Contribute to a climate that provides residents with the highest practical level of physical, mental, and psychosocial well-being and quality of life.
- Assumes a social work caseload of all residents on a nursing unit in accordance with the Social Services Manual policies.
- Greets and helps acclimate new residents and assists with development and review of plans of care to meet long- and short-range health care goals.
- Completes required forms for admissions and discharge processes; participates in discharge or transfer decisions as applicable.
- Documents pertinent information according to department and facility policies; cooperates with other departments to meet residents\' physical, emotional and social needs.
- Provides financial, personal, or group counseling for residents and their family members or sponsors; participates in in-service training programs when requested.
Qualifications
- Minimum of a bachelor\'s degree in social work or a related field (human rehabilitation counseling, psychology) from an accredited institution; experience in sociology, gerontology or related areas may be included.
- One year of supervised social work experience in a health care setting, working directly with individuals.
- Experience in casework is required.
- Valid driver\'s license and clean driving record.
- Experience in admission or discharge areas of social work is desirable.
- Desire and ability to work with the elderly and cooperate with other staff members.
- Must be able to read, write, speak, and understand English at an intermediate level or higher.
