Overview
Social Worker – Adult Day Health Program (Day Shift). Licensed Certified Social Worker (LCSW) or Licensed Independent Clinical Social Worker (LICSW) needed to support adults with physical, cognitive, and/or psychosocial impairments in an Adult Day Health Program. Role focuses on helping clients live as independently as possible while providing respite for caregivers.
Shift: AM – 7.5-hour shifts | Monday–Thursday: 8:00 AM–3:15 PM, Friday: 8:00 AM–3:00 PM
Pay: $55.00–$63.00 per hour
Schedule: 36 guaranteed hours/week | No weekends | No major holidays | Closed on Thanksgiving & the Friday after Thanksgiving | Open on Indigenous Peoples’ Day (10/13)
Responsibilities
- Manage full caseload for the Adult Day Health Program, including new client referrals, case management, and family support.
- Provide 1–2 hours per day of direct client interaction in addition to administrative and casework duties.
- Complete required documentation within 2 weeks of admission and every 90 days thereafter.
- Collaborate with healthcare team members (nursing, MD, case management) to coordinate care.
- Assist clients with ambulation and transfer techniques.
- Lead or assist with small and large group programs.
Qualifications
- Licensure: MA LCSW or LICSW.
- Certification: BLS
- Experience: Minimum 2 years post-MSW, with knowledge of the social work process, community resources, and relevant agencies (DMH, DDS, ASAPs).
- Strong time management, multitasking, and communication skills.
- Ability to work effectively with diverse populations and varying disabilities (physical, cognitive, mental health).
Preferred Qualifications
- Experience in community-based healthcare settings.
- Knowledge of community support services (transportation, Meals on Wheels, housing support).
- Experience coordinating with external care teams and case managers.
- Familiarity with EPIC registration and scheduling systems.
