The Social Worker is a member of the core interdisciplinary team responsible for assessing psychosocial symptoms of patients and families, developing outcomes to address these symptoms, and providing psychoeducation, supportive counseling, and casework interventions to meet these outcomes. The Social Worker also offers psychosocial education and consultation to the IDG.
Responsibilities:
- Participates in preparing, evaluating, and executing the interdisciplinary care plan to ensure high-quality psychosocial care, continuity, and comprehensiveness.
- Performs bio-psycho-social-spiritual assessments of patients and families, focusing on needs rather than diagnostics.
- Provides individual and family supportive counseling.
- Refers patients and families to appropriate community resources.
- Maintains accurate and timely documentation in the patient’s medical records.
- Attends weekly team meetings to plan care.
- Participates in patient case conferences.
- Provides psychosocial education and consultation to the core team.
- Participates in psychosocial supervision meetings and other training sessions; may supervise other psychosocial team members, including interns, under the team manager's supervision.
- Acts as coordinator and communicator for volunteer interventions within the care plan.
- Provides bereavement care, including group activities.
- Participates in psychosocial spiritual on-call rotations as scheduled.
Job Requirements:
- Ability to travel as required.
- Thorough knowledge of community resources or willingness to acquire this knowledge.
- Understanding of key psychosocial issues faced by terminally ill patients and families, including effective intervention strategies.
- Ability to work effectively as part of an interdisciplinary team.
- Excellent listening and communication skills.
- Ability to advocate effectively for patients and families within the care team and community agencies.
Qualifications:
- Master’s of Social Work from an accredited school, or grandfathered in as per the 2008 Medicare Hospice Conditions of Participation.
- At least one year of social work experience in a healthcare setting.
- Experience working with terminally ill patients and families is preferred.
- State licensure and/or certification as required.

- GA: MSW supervised by LCSW until LMSW obtained

- WI: Social Worker Certificate
