What to expect in this role:

You will provide therapeutic intervention and social work services to patients and their families to enhance comprehensive, integrated, and uninterrupted care in the hospital and to facilitate continuity of care in the community. Responsibilities include conducting patient assessments, developing care plans, coordinating with providers, and serving as a patient advocate.

The Social Worker collaborates with psychiatrists and interdisciplinary teams to develop and manage treatment plans. They serve as liaisons between the hospital, community, and insurers to facilitate access to clinical treatment. Services include bio-psychosocial assessments, discharge planning, individual and family education, group therapy, crisis intervention, and more. Social workers also participate in committees and may serve as field instructors for interns.

Education/Experience Required:

• Graduation from an accredited school of social work with a Master’s Degree.
• Clinical knowledge and analytical skills to develop effective care plans.
• Ability to perform a wide range of professional social work services.
• Effective communication skills with patients, families, and hospital personnel.

Certifications/Licenses Required:

• Current Licensed Master Social Worker.
