The Social Worker is responsible for helping residents adjust to their new environment, providing counseling and support, and connecting them with community resources. In this role, you will advocate for residents’ rights and well-being, collaborating with the Sales & Marketing team, Wellness Center Director, and Resident Services Director to ensure resident needs are met within the facility and beyond.
Responsibilities
- Assess resident needs: Conduct psychosocial assessments to identify resident needs, including emotional, social and practical needs, as well as any special requirements.
- Provide counseling and support: Offer individual and group counseling to help residents adjust to living in an independent living facility and cope with any challenges they may face; maintain case notes and files.
- Connect residents with resources: Link residents with relevant internal and external community resources, including healthcare providers, financial assistance programs, transportation services, and social activities.
- Advocate for residents: Ensure respect for resident rights and that resident voices are heard in care planning and decision making; serve as standing member of Aviva for Life committee.
- Facilitate communication: Help foster communication between residents, their families, facility staff, ensuring everyone is informed and involved in supporting the residents.
- Continuum of care planning: Assist with planning if a resident needs to transition to a higher level of care, coordinating with other providers and support services to ensure a smooth transition.
- Educating staff and families: Provide education regarding relevant regulations, resident rights, and the importance of addressing psychosocial needs.
Educational/Professional Qualifications
- Licensed Clinical Social Worker, minimum 2 years’ clinical experience preferred
- Experience with senior population is a plus
Knowledge, Skills And Abilities Required
- Working knowledge of community resources.
- Demonstrated knowledge and experience working with individuals experiencing major life transitions.
- Possess good clinical, counseling, and organizational skills.
- Able to concentrate and be detail oriented; possessing excellent communication and listening skills.
- Demonstrated ability to be open, sensitive, flexible, and non-judgmental.
- Ability to work as a member of an interdisciplinary team where roles may be diffused and/or overlapping.
- Must be willing to work flexible hours to meet resident/family needs.
