As a hospice medical social worker, you will conduct psychosocial evaluations and provide ongoing counseling to patients and families during their end-of-life journey. Working in accordance with the care plan, you will offer emotional support when it is needed most.

Qualifications :

• A heart to serve patients and families and a passion for providing the best possible care
• Education: MS degree in social work from an accredited school approved by the Council on Social Work Education
• Licensure: Current state license as a social worker
• Experience: 2+ years of clinical work, preferably in healthcare or hospice
• Required: Reliable transportation. Ability to sit, stand, bend, move intermittently, lift at least 25 lbs, and bear the weight of an average adult effectively.
