Are you a compassionate, skilled communicator who’s passionate about improving the lives of seniors? We’re looking for a Social Worker to join our caring team and support both long-term care and short-stay residents in our nursing home.

As a Social Worker in our facility, you'll play a key role in helping residents and their families navigate care transitions, emotional challenges, and support services. This is a unique opportunity to make a meaningful difference in the lives of both short-term rehab patients and those requiring long-term support.

What You’ll Do:

• Provide emotional support and counseling to residents and families
• Coordinate care plans in collaboration with our interdisciplinary team
• Facilitate smooth admissions, transfers, and discharges
• Advocate for resident needs and preferences
• Connect residents with community resources and services
• Ensure documentation complies with regulatory standards

• A caring, empathetic personality with a genuine desire to help others
• Strong communication and interpersonal skills
• Ability to handle sensitive situations with professionalism and compassion
• Bachelor’s or Master’s degree in Social Work (BSW or MSW)
• 2-3 years experience in a nursing home or healthcare setting preferred
