Responsibilities
- Provide primary therapy to patients 18 years and older admitted to the Adult Partial Hospital Program (PHP).
- Conduct psychosocial assessment, treatment, and discharge planning for patients on assigned caseload.
- Provide crisis intervention, maintain the therapeutic milieu, and assess/manage patient safety.
- Lead therapeutic groups and conduct intake assessments as appropriate.
- Develop and maintain relationships with community agencies and providers who refer to and receive patients from the PHP.
- Participate on a multidisciplinary team for ongoing case review.
Responsibilities (additional)
- Provide coverage for therapists in clinic per diem, including psychosocial assessments, behavioral health treatment, and education to patients and families (individual, family, and/or group therapy).
- Coordinate activities related to comprehensive treatment and discharge planning for patients in caseload.
- Collaborate with interdisciplinary treatment teams; request and coordinate psychiatric consultations as indicated; secure insurance authorization for treatment when appropriate.
- Link patients to community resources and provide backup coverage; cover other units/programs as needed.
- Document treatment in the patient’s electronic medical record in compliance with policy.
Qualifications
- Master’s degree from an accredited school of social work (CSWE) required.
- 2–3 years of professional social work practice beyond the Master’s degree, including psychiatric emergency, crisis, or outreach work; or an equivalent combination of education and experience with adult populations. Prior experience in individual or group therapy in an acute clinical setting is preferred.
- Knowledge of behavioral health assessments/evaluations and evidenced-based interventions preferred.
- Commitment to a biopsychosocial approach to care for individuals and families preferred.
- Dedication to a team-based integrated model of care preferred.
- Demonstrates cultural humility and anti-racism values; strives to create a welcoming environment for all patients.
- Licensure as a social worker is required by the State Board for Social Work, State Education Department, Division of Professional Licensing Services. If not obtained upon employment, must be obtained within 18 months (or per OMH guidelines for certain roles).
