What You'll Do

  • Advocate for the well-being of residents and ensure their holistic needs are met, contributing to an improved quality of life.
  • Conduct comprehensive psychosocial assessments to identify residents' social, emotional, and environmental needs, strengths, and preferences.
  • Collaborate with residents, families, and the interdisciplinary team to develop individualized care plans addressing social, emotional, and psychological concerns.
  • Provide counseling and emotional support to residents and families, offering guidance on coping strategies and adjustment to care transitions.
  • Advocate for residents' rights and preferences to ensure their voices are heard in care planning and decision-making processes.
  • Connect residents and families with community resources, support services, and advocacy organizations to address social, financial, and legal needs.
  • Provide crisis intervention and support during challenging situations, such as grief and loss, family conflicts, or changes in health status.
  • Collaborate with the team to ensure services align with person-centered care.

Qualifications

  • Current licensure or certification as a Social Worker in the state of employment
  • Previous experience in Long-Term Care, geriatrics, or a related field
  • Strong assessment, counseling, and crisis intervention skills
  • Excellent communication and interpersonal abilities
  • Ability to work collaboratively as part of a multidisciplinary team
  • Empathy, compassion, and a commitment to person-centered care
