POSITION DESCRIPTION: Assures continuity of care during the transition from the hospital to post-hospital care by assisting patients and their families with personal and environmental difficulties as they transition from CPH to home or other health care providers.

REQUIREMENTS:

1. Bachelor Degree in Social Work, MSW preferred
2. State of AK Social Worker Licensure or eligibility to obtain upon hire
3. BLSP certification within 30 days of hire
