Responsibilities

• Seeking a Social Worker - LCSW or LICSW to provide home and community-based early intervention services to infants and toddlers at risk for developmental delays and their families.
• Responsibilities include conducting assessments, developing individualized plans (IFSP), delivering child development interventions, and acting as a service coordinator to ensure family needs and goals are met.
• Collaborate with a multidisciplinary team, support families through coaching and advocacy, and document progress accurately.

Qualifications

• Requirements include a Master’s in Social Work or licensure (LCSW/LICSW), experience with young children and families, strong communication skills, cultural humility, and reliable transportation. Bilingual skills are a plus.
