Social Worker Job at SBH Health System in New York

As part of the Medicaid Health Home Program, this position provides social work case management support to an assigned panel of patients with complex behavioral and medical needs. The Social Worker will utilize acceptable social work practices in providing psycho-social support to patients with identifiable social work needs, including responsibility for management of HARP and Health Home patients. This will include comprehensive assessment, plan of care development, referral facilitation, and acting as liaison with relevant clinical and social service providers.

Requirements

• Education Required: Master of Social Work
• Required Licensure: NYS Social Work License
• Experience/Skills Required
• Minimum of three years of experience in relevant care coordination service delivery.
• Prior experience in the Health Home model preferred.
• Proficiency in electronic data collection systems.
