Description
Position Summary:
Responsible for ensuring that clients receive the individual range of medical care and other comprehensive services necessary to optimize each client's substance use, mental health, and health outcomes.
Essential Job Functions:
- Responsible for the social evaluations of all Project participants.
- Participate in the initial interview for the determination of needs and maintain the social reports of each participant, carrying out assessments of an emotional nature, self-esteem, and independent living capacities.
- Document the participant's social history.
- Assess participants' needs in conjunction with the team and other peers.
- Manage support groups and carry out service activities on a daily basis, providing individual and group therapies aimed at mastering social and community skills.
- Provide guidance related to the search for housing and employment services.
- Contribute to the development and increase of Social Welfare, involved in individual, group, and community services related to various Areas of Welfare and Social Services.
- Coordinate and apply all socio-health resources to the client's reach.
- Assist in the integral development of clients and their families in collaboration with the team.
- Hold self-help meetings with the participant's family and/or partner.
- Review records and ensure compliance with participant eligibility documents, in coordination with the Coordinator and Case Managers.
- Keep the Coordinator informed of situations that occur during interventions.
- Represent and accompany clients to court hearings.
- Set new goals for each participant to develop a life plan and reintegrate into the community by implementing the skills learned within the help center.
- Perform other tasks as assigned by the Supervisor.
- Pursuant to Executive Order 2021-075, you must submit proof of vaccination against COVID-19 and, pursuant to Order 2021-082, evidence of vaccine booster; or a medical or religious exemption that meets the requirements set forth in the executive order, as well as a negative PCR test or antigens every 7 days.
Qualifications
Education:
Bachelor's or Master's Degree in Social Work, Licensed Social Worker, and active member of the Social Workers Board, with 2 to 5 years of experience, knowledge, and evidence-based practices. Computer knowledge is required.
