This is a clinical position requiring in-depth knowledge of mental health, DSM 5-TR diagnostic criteria, and skills in crisis intervention and stabilization. As part of the ACT, you must be willing to go out into the community to provide services. This position requires clinical experience and a desire to work as part of a team.

Essential Functions:

• Conduct daily visits in the community, with documentation in accordance with agency and OMH standards.
• Complete Wellness Plans, assessments, risk assessments, and other required documentation.
• Provide individual psychotherapy using evidence-based treatment modalities.
• Participate in daily meetings and inform the ACT team about family issues, incorporating findings into service plans.
• Involve family and significant others in treatment, providing psycho-education and relapse prevention strategies.
• Act as advocate and liaison for consumers with family, support systems, landlords, and community supports.
• Provide 24-hour crisis intervention on-call services on a rotating basis.
• Perform other duties as directed by the Team Leader.

Qualifications:

• Master’s Degree required - LMHC, LMSW, LCSW, or equivalent licensure in NYS.
• At least one year of experience working with individuals with serious mental illness and their families.
• Outcome-oriented in a fast-paced environment with deadlines.
