Work Schedule: Full-time. Mon-Fri; 8:00 AM to 4:30 PM

Responsibilities

  • Responsible for providing case management for a caseload of veterans in the HUD-VASH program with supervision. The incumbent reviews agreements between Section 8, landlords, Public Housing Authorities, and the VA in establishing appropriate referral sites for HUD-VASH placements.
  • Performs site visits of HUD-VASH housing to ensure that veterans reside in safe environments in compliance with local housing codes.
  • Verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation.
  • Responsible for ensuring that homeless veterans are considered as primary participants for program acceptance, with supervision oversight.
  • Collects and submits HUD-VASH Program participant data as outlined by NEPEC and/or HOMES evaluation procedure.
  • Complies with criminal conflict of interest laws and Executive Branch Standards of conduct to avoid conflicts of interest while serving as the HUD-VASH Case Manager.
  • Actively participates in the HUD-VASH Program conference calls and attends training offered by the HUD-VASH.
  • Follows local policy and Standard Operating Procedures for the HUD-VASH Program, including documentation requirements for CARF and Joint Commission accreditation.
  • Participates in program planning and development, advising the supervisor of shifting trends with caseloads and identifying needs for program procedure or policy changes.
  • Improves professional skills, participates in staff development projects, seminars, and training programs as requested.
  • Performs accountability functions: document all contacts with patients/families in medical charts; completes statistical and other reports per VA Medical Center policies.
  • Works with VA Medical Center multi-disciplinary teams and community agencies to develop more effective and efficient ways of delivering healthcare services to HUD-VASH Program veterans.
  • Completes duties including: psychosocial assessments, active participation in treatment planning and discharge, individual/family/group counseling, case management, and education on community resources, advance directives, and VA benefits; may supervise social work students; participates in committee assignments as needed.

Qualifications

Basic Requirements:

  • You must be a U.S. Citizen to apply for this job.

  • Must be proficient in written and spoken English.

  • Must pass pre-employment driving physical examination.
  • Participation in the seasonal influenza vaccination program is a requirement for all Department of Veterans Affairs Health Care Personnel (HCP).

Education:

  • Master's degree in social work from a CSWE-accredited program. Verification via CSWE site.

Licensure:

  • Must be licensed or certified by a state to independently practice social work at the master's degree level for GS-0185 series; some exceptions apply at GS-9 level; see text for details.

Experience and Demonstrated KSAs (GS-9/GS-11):

  • GS-9: Demonstrated knowledge and abilities include veteran-centric interventions, psychosocial assessment, treatment planning, individual/family/group therapy, and building working relationships; knowledge of medical/mental health diagnoses and terminology.
  • GS-11: Requires one year of post-MSW experience at the GS-9 level (VA or non-VA) and independent practice licensure/certification, or a doctoral degree substituting for one year of experience. KSAs include knowledge of community resources, psychosocial assessments, treatment planning, various treatment modalities, and consultation to new staff.
