In this role you will be responsible for providing services to respond to the emotional needs of residents and their families. The position also assists with resident admission and the referral process. This position reports to the Administrator.


Responsibilities



  • As part of the admission process, meet with the resident and family to review policies on advanced directives, resident rights, and discrimination.

  • Conduct and document a social services evaluation, including identification of resident problems/needs.

  • Provide/arrange for social work services as indicated by resident/family needs.

  • Assist residents, as needed, with financial planning.

  • Keep the resident's family informed of resident problems, personal needs, transfers, and changes of level assignment.

  • Maintain a current list of community resources to facilitate referrals.

  • Apply professional standards of social work practice and establish criteria to assure that services provided meet established quality standards.

  • Demonstrate satisfactory interpersonal skills to interact with residents, families, community and interdisciplinary staff.

  • Demonstrate assessment skills sufficient to evaluate residents' behavior, to collect data to evaluate psychosocial needs, risk factors for psychosocial deterioration and residents' responses to interventions. Able to interpret and explain resident behavior to staff in a way that fosters understanding, facilitates treatment and respects resident rights and minimizes use of chemical and physical restraints.

  • Implement social service interventions that achieve treatment goals, address resident needs, link social supports, physical care and the physical environment to enhance quality of life.


Qualifications



  • Level of Formal Education: A Bachelor’s Degree from a four-year college or university; or one to two years related experience and/or training; or equivalent combination of education and experience.

  • License: LSW

  • Area of Study: Social Services Preferred

  • Years of Experience: 2+ years

  • Type of Experience: Experience in social services, experience in gerontology is a bonus

  • Technical Competencies: Basic computer skills

  • Ability to effectively communication and actively listen.

  • Multi-tasking skills, and good organizational skills.

  • Personal Attributes: Professional demeanor, empathy
