This position located in the Department of Social Services performs professional, administrative, and direct duties involved in assisting individuals and families who may experience increasingly complex and diverse needs as well as crisis management. The position works with all populations, with the exception of minors, including homeless individuals and families and those at risk of becoming homeless. Metro Social Services also assists customers with navigating the system and obtaining the necessary information, documents, resources, and case management services as well as working to find and secure housing opportunities for the customers. Must be self-directed, show initiative and be comfortable working independently and as a team.
Metropolitan Social Services is a trauma informed care organization who is in the business of assisting Nashvillians who are experiencing hardships. Service areas include but may not be limited to counseling, information, and referral, short- and long-term case management, home and community-based services, mental health services and other assistance. Responsibilities
- Perform concise, comprehensive social assessments and developing service plans based on the customers' individual needs, assessments of individuals which may include individuals with acute or persistent mental disorders or co-occurring disorders.
- Provide information and referral services to customers and the public that enables them to utilize the programs' services to best address their needs.
- Provide short-term or long-term case management services to all populations served.
- Conduct home visits to customers' homes and/or other agencies/locations within the community and/or designated co-located partnership sites.
- Research and advocate for customers to receive appropriate services through community resources such as public assistance programs, drug rehabilitation programs, and housing shelters.
- Coordinate services, plan, and maintain ongoing contact with customers for completion of services.
- Enter data into multiple case management systems and computer applications.
- Submit monthly reports.
- May be responsible for providing basic care, counseling, and assistance to individuals with mental illness or developmental disabilities.
- Work with various established Metro partnerships as well as being flexible to work across the Division of Programs in any of the agency identified program units (Family Support, Mental Health, Home and Community Based Services).
- May perform case management and case review of assigned Conservatorship cases.
- May provide written summaries and any reports from completed interviews from the assigned Conservatorship cases.
- Build and maintain working relationships with various Metro Departments and other community agencies to ensure the emotional and social well-being of the assigned constituents are being met, assessed, and documented on a regular basis.
- May occasionally work Overflow Shelter shifts.
- Attend community events virtually or in-person.
Minimum Qualifications
Bachelor's degree in Social Work from an accredited college or university.

LICENSE REQUIRED:
Valid Driver License.

Per Metro Ordinance No. SO94-1078. All employees of the Metropolitan Government shall be residents of the State of Tennessee or become residents of the state within six (6) months of employment as a prerequisite to employment with the Metropolitan Government.
Experience Requirements
- Two (2) or more years of professional social work or case management experience in the mental health, domestic violence and/or substance abuse field.
- Experience working with diverse populations including the homeless and individuals with mental health concerns.
- Skilled in interviewing, making case assessment, counseling, and assisting clients in goal formulation and realizations and case referrals.
- Experience or knowledge of Domestic Violence, Mental Health, Home and Community Based Services, Homeless, Trauma, Elderly & Disabled, Veterans and the issues.
- Experience and/or knowledge of the Two-Generation approach, Advanced Childhood Effects (ACES) and Trauma Informed care.
