Job SummarySocial Workers provide individual, family and group psychotherapy to patients diagnosed with a wide range of mental illness. They maintain documentation in compliance with regulatory bodies.
Essential Functions
- Complete intake evaluations and provide initial diagnosis and case conceptualizations for newly admitted patients
* Be able to conduct, document and present comprehensive psychosocial evaluations and do an appropriate diagnostic interview with knowledge of DSM V diagnostic criteria - Complete initial treatment plans and treatment plan updates every 90 days
* Be able to document treatment plans within Joint Commission and Office of Mental guidelines that are aligned with the patient's goals and their DSM V
psychiatric diagnosis - Completes documentation of clinical visits within 72 hours of the appointment
* Completes paperwork and timely and within compliance standards - bills appropriately for services provided - Appropriately conducts outreach for patients, and conducts appropriate discharging planning within Office of Mental Health Guidelines
* Closes cases within 90 days, and conducts appropriate outreach, facilitating coordination with alternate care as indicated - Functions independently but utilizes supervision, consultation and shared with their interdisciplinary team as appropriate
* Attend supervision, and reach out for supervision for consultation as clinically indicated - Practices and Conduct Professional Work Social Maintains
* Ensures confidentiality in accordance with hospital and OMH standards Participates in continuing education in order to provide high quality care Practices within the legal and ethical standards of NY State Mental Health law - included mandated reporting and SAFE ACT reporting - Participates in departmental programs related to quality improvement, staff meetings, continuing education, risk management, diversity and inclusion
* Attends all meetings and participates appropriately. - Demonstrates skills and knowledge related to the identification, reporting and appropriate interventions for survivors of individual and family violence, elder abuse and child abuse and neglect.
* Knows reporting requirements and upholds mandated reporter responsibility - Conducts crisis interventions, and assesses and intervenes safely based on patient's risk to self, others or risk of self- neglect or other dangerousness
* Associate will appropriately assess and intervene to crisis, and reach out for consult and supervision as required. Risk will be managed within Department of Psychiatry guidelines.
Qualifications
- MS Required
- 1-3 years LCSW
- Operates personal computer and phone.
- Licensed Master Social Worker
