Licensed Practical Nurse (LPN) - Long Term Care
Job Overview
We are seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team in providing exceptional care in a long-term care environment. This position offers a unique opportunity to work with geriatric residents, delivering care with dignity and respect.
Shift Details

• Schedule: 40 hours/week
• Shift: 5x8.0 (Days/Evenings)
• Availability: Weekends and Holidays required
• Day Shift: 7am - 3pm
• Evening Shift: 3pm - 11pm

Responsibilities

• Provide direct patient care to a designated group of residents.
• Administer medications and oversee the care provided by Certified Nursing Assistants (CNA).
• Ensure proper documentation of patient care in compliance with facility policies.
• Collaborate with a multi-disciplinary team to ensure optimal patient outcomes.
• Utilize good critical thinking skills to assess patient needs and respond appropriately.
• Foster a respectful and dignified environment for all residents.

Qualifications

• Current New York State LPN license is required.
• BLS certification from the American Heart Association is required.
• Minimum of two years of experience in a long-term care setting.
• Ability to work independently and as part of a team.
• Strong communication and interpersonal skills.
